HERE IS A LIST OF DOCUMENTS WE ASK YOU TO PREPARE FOR OUR FIRST MEETING

1. Company provided group benefits for you and your spouse. (If a print-out of specific coverages and benefits are available, please include those)

2. A monthly budget of your expenses

3. Tax returns for the last 2 years

4. Wills and trust documents

5. Mutual Funds/Stock/Bond etc. statements (Preferably the recent year end)

6. Bank statement-listing accounts

7. Any pension or 401(k) plan statements

8. Recent Social Security Statements

9. Copy of recent Mortgage Statement

10. All insurance policies
